KARBAPENEMLERE D?RENÇL? KLEBS?ELLA PNEUMON?AE ?ZOLATLARININ APRAM?S?NE DUYARLILIKLARININ ARA?TIRILMASI

Amaç: Bu çal??mada, çe?itli klinik örneklerden izole edilen karbapenemlere dirençli Klebsiella pneumoniae izolatlar?nda blaOXA-48 benzeri, blaCTX-M-15, blaNDM-1 genlerinin varl??? ve apramisine duyarl?l?klar?n?n ara?t?r?lmas? amaçlanm??t?r. Gereç Yöntem: Çal??maya Haziran-Eylül 2020 tarihleri aras?nda ?stanbul T?p Fakültesi Hastanesinde yatan hastalara ait 12 ço?ul K. izolat? dahil edilmi?tir. izolatlarda imipenem, meroperem, ertapenem apramisin için minimum inhbisyon konsantrasyon (M?K) de?erleri, EUCAST önerilerine göre s?v? mikrodilüsyon yöntemiyle saptanm??t?r. Antibiyotik konsantrasyonlar? (imipenem, meropenem, ertapenem) NARMS (apramisin) s?n?r de?erlerini kapsayacak ?ekilde <0,016-256 µg/mL olarak belirlenmi?tir. Klasik polimeraz zincir reaksiyonu (PCR) blaOXA-48, blaNDM-1, blaCTX-M-15 ara?t?r?lm??t?r. Bulgular: Karpanenemlere izolatlar?n?n tamam?nda (n=12) benzeri genleri, bir izolatta (%8,33) geni ?zolatlar?n?n tümü ertapeneme, %91,7’si meropeneme %58,3’ü imipeneme bulunmu?tur. Tüm izolatlar?n duyarl? oldu?u belirlenmi?tir. M?K aral??? 4-256 µg/mL, meropenem 0,5-64 imipenem 0,5-128 <0,016-2 aral???nda Sonuç: dü?ük de?erlerinin saptanmas?, karbapenemlere veya tüm antibiyotiklere izolatlarda, tedavide alternatif kullan?labilecek ajan olabilece?ini dü?ündürmektedir. nedenle apramisinin klinikte kullan?labilmesiyle ilgili daha kapsaml? çal??malara ihtiyaç duyulmaktad?r.
